Who Do I Complain to About a Restaurant or Report a Food Born Illness?

If you've found unsanitary conditions at a Restaurant Facility within Dallas County or contracted a Food Born Illness after eating at one, click on the below icon to download our complaint form and fill it out.

Additional complaints regarding health issues with Lodging, Day Care and Schools Facilities will be handled in the same manner. Reports on health issues also encompass complaints with Septic Issues. All complaints will be investigated by our Environmental Public Health Specialist at our office.

Please Note! All Cities and Counties have health problems, some serious and others not, you may want to take this into consideration before filing a complaint and forwarding it to the Dallas County Health Department.
