2. Proof of
Income - This includes all household income
received such as wages, SSI, SSD, retirement pension, child
support, food stamps and any temporary assistance. This
does not mean just one parents income. This includes both
parents or whomever you live with and their income. If you
claim no income you must have documentation of some type of
income support. Check stub, tax return, letter
from employer, letter from Social Security, Court Order for
child support, statement from DFS for food stamps or TANF

4. Proof of Missouri Residency - Proof of
residency does not have to be in your name as long as it is one
of the following items and shows your current address:
phone bill, electric bill, rent or mortgage statement, written
statement from a third party you are living with and must
include current address
