Chronic Disease Primary Prevention
Nearly 7 out of 10 Missourians who die each year will die of a chronic disease. Chronic diseases, such as cardiovascular disease (primarily heart disease and stroke), diabetes, and cancer, are among the most common and costly, yet most preventable of all health problems. Prolonged illness and disability from chronic diseases such as diabetes, arthritis, and asthma result in a decreased quality of life for many Missourians.

Dallas County Health Department along with partners Polk County Health Department, Hickory County Health Department and Dade
